Penile filler in Los Angeles with a urologist
Penile filler adds girth without surgery. I place hyaluronic acid gel, the same kind used in lip filler, under the skin of the shaft in a short office visit under local anesthetic. It starts with a free, private consult for men in Los Angeles and Beverly Hills, and by video anywhere in California or Georgia.

What penile filler is
Penile filler is hyaluronic acid, the same FDA-approved gel used in lip and facial filler. I lay it in a thin, even layer under the skin of the shaft. It sits between two tissue layers called the dartos fascia and Buck’s fascia, outside the erectile chambers, so it adds girth and leaves the parts that make an erection work alone.19
Nothing is cut and nothing is sewn in. Because it’s hyaluronic acid, it can be adjusted or dissolved with an enzyme called hyaluronidase if it ever needs to be.12 Permanent fillers like silicone and PMMA can’t be dissolved.
When men ask me what to compare it to, I tell them to think lip filler 15 years ago. That’s about where penile filler is today. The idea is the same as lip filler. The difference is the anatomy, and the training it takes to work in it safely.

Who it’s for
Most of the men I see for filler are in their 40s and 50s and in a long relationship. They’ve noticed a change with age, or after a partner’s pregnancies. Many are on testosterone or thinking about it. They want some fullness back and a bit more confidence.
A good fit
- An adult man with normal anatomy and erections that work
- A clear goal of more girth and a fuller look
- Fine with a temporary result that fades over 12 to 18 months
- Noticing a change with age or over years with a partner, or wanting more confidence
Not yet, or not for you
- An active genital or urinary infection
- A bleeding disorder, or blood thinners that can’t be paused
- An allergy to hyaluronic acid fillers or lidocaine
- Permanent filler (silicone, PMMA, paraffin) already in the area
- Untreated Peyronie’s disease or another penile condition, which I treat first
- Worry about size that’s out of proportion to the exam
Why I screen younger men carefully
Men under 30 get a careful screening before anything else. In 2024 the Sexual Medicine Society of North America said penile dysmorphic disorder has to be ruled out before any invasive treatment.10 Some of the men who worry most about size are completely normal, and filler won’t fix that worry. If that’s you, I’ll tell you.
Uncircumcised men get extra counseling too. A longer foreskin raises the chance of swelling, or of gel shifting forward after treatment.5
In greater Los Angeles about seven people do penile filler, and two of us are urologists. Most of the rest are plastic surgeons who dabble in it.
Why a urologist should do your filler
Before Los Angeles I spent two years doing urologic surgery in South Georgia, kidney stones and cancer with a focus on men’s health. That’s where I started doing penile filler.
- The anatomy
- The dorsal nerves and blood vessels, the urethra and the layers of fascia are where a urologist operates. Knowing which plane the gel belongs in is most of the job.
- Checks first
- I look at erections, curvature (Peyronie’s disease), hormones and urinary symptoms before we talk about girth. Sometimes the right fix is something else.
- Technique
- Published protocols use a blunt cannula through one small entry point at the base. The gel is laid in evenly and then molded smooth.1,6
- Afterward
- If a lump, unevenness or migration shows up, I’m the one who treats it, with hyaluronidase when it’s needed.
The rare serious complications in the medical literature mostly involve men injecting themselves, unknown products, or gel placed in the wrong tissue.17,18
How the procedure goes
It’s a 15 to 20 minute office procedure under local anesthetic. There are no incisions and no general anesthetic, and you walk out the same day.

- The consultFree, by phone or video
- A private call or video visit with me. We go over your goals, your history and your questions. If filler isn’t right for you, I’ll say so.
- The examIn Los Angeles, from December
- A hands-on urologic exam. I check your anatomy, your erections and anything that should be treated first.
- NumbingLocal anesthetic
- Local anesthetic (lidocaine). You’re awake, comfortable and able to talk the whole time, and most men find it almost painless.
- Placement15 to 20 minutes
- Through one small entry point at the base, I lay the gel evenly under the skin of the shaft with a blunt cannula and mold it smooth.6
- HomeThe same day
- You go home with a light dressing, simple massage instructions and a pause on sex while the gel settles. Published protocols say about three weeks.6
- Follow-upTop-ups at 12 to 18 months
- A check-in to look at the result. A small touch-up evens things out if needed, and a top-up at 12 to 18 months keeps it.9
Recovery
Swelling, tenderness and some bruising are normal at first and fade as the tissue settles. Keep the dressing dry, follow the massage instructions, and hold off on sex for as long as I tell you. The massage is part of the treatment, because it helps the gel settle evenly.
If anything looks or feels wrong, call. Most early problems, like a small lump or an uneven area, are simple to fix when they’re caught early, and I’m the one who fixes them.
In studies, men gain about 2 to 2.5 cm of girth.
In published studies men gained on average about 2 to 2.5 cm of flaccid girth, roughly 3/4 to 1 inch around, and you see the change the same day.
The evidence comes from randomized trials and larger cohorts. A meta-analysis of 283 men found hyaluronic acid gave more girth than polylactic acid filler, with better sexual satisfaction.7 In the original 50-man study, midshaft girth went from 7.5 cm to 11.4 cm at one month.1
The erect gain is smaller. In a year-long study it was about 1.3 cm at one month and 0.8 cm at twelve months.4 Men like the result. In an audit of 324 men, 89% were satisfied,6 and in a 2026 study appearance satisfaction improved in 80% of men and sexual satisfaction in 85%.9
What men tell me afterward
Men come back and tell me two things. Sensation is stronger, and erections are harder. The way I explain it, the filler stretches the skin and its nerve fibers, and its gentle pressure on the erectile chambers means less blood is needed to get hard.
How long it lasts
Hyaluronic acid is temporary. The body breaks it down slowly, so the result fades gradually over roughly 12 to 18 months. One 230-man study kept about two-thirds of the one-month gain at six months.5 Another kept about 70% of the flaccid gain at a year,4 and the original study saw little loss out to 18 months.1
So I plan touch-ups from day one. A 2026 study found that planned touch-ups are what keep results long term.9
Does it add length?
Yes, when you’re soft. The added volume gives a fuller, longer hang, and I tell men it makes growers become showers. For men who have lost some length over the years it’s a great option. Erect length is set by the erectile tissue. If that’s your main goal, I’ll walk you through every option, implants included.

Risks, side effects and reversibility
Hyaluronic acid filler has a low rate of complications, and most are minor and fixable. A review of 13 studies and 1,311 men concluded that it “seems to be safe, efficient, and with a high satisfaction score.”8 In a 301-man comparison, local side effects were 7.2% with hyaluronic acid, 11.9% with polylactic acid and 14.3% with PMMA.11
Serious complications are rare. The published cases mostly involve men injecting themselves, unknown products, or gel put into the glans (the head), where the blood supply is different.17,18 Choose who injects you as carefully as you’d choose a surgeon.
Reversing it
An enzyme called hyaluronidase dissolves hyaluronic acid. I can use it to fix unevenness, lumps or migration, or to take the filler out entirely for the rare man who wants it gone.6,12 Permanent fillers like silicone and PMMA only come out with surgery.

How filler compares with PMMA, implants and fat
Search for penile enlargement in Los Angeles and you’ll find everything from injections to implants. These are the main options, side by side.
| Option | What it is | Reversible | Trade-off |
|---|---|---|---|
| Hyaluronic acid fillerWhere I start | Temporary gel under the skin of the shaft. Office visit, local anesthetic. | Reversible: Yes, with hyaluronidase | Fades over 12 to 18 months, so it needs top-ups. It’s the smallest commitment and has the lowest rate of local side effects. |
| PMMA (for example Bellafill) | Semi-permanent microspheres injected under the skin. | Reversible: No, it needs surgery | More girth in one study (3.5 cm), but about half of men noticed surface irregularities.13 It had the smallest gain in satisfaction in a 301-man comparison.11 |
| Silicone, paraffin or mineral oil | Permanent injections, often from the black market. | Reversible: No | Injectable silicone isn’t FDA-approved for any aesthetic procedure,16 and the SMSNA strongly recommends against permanent fillers.10 Granulomas and disfigurement can follow. |
| Fat transfer | Your own fat, taken by liposuction and injected. | Reversible: No | The American Urological Association says fat injection for girth has not been shown to be safe or effective.15 Uneven reabsorption and lumps are common. |
| Penuma (silicone implant) | A silicone sleeve implanted under the skin of the shaft. | Reversible: Removal is surgery | Added 3.1 cm of girth in 92 men, and 12% had the implant removed.14 It’s surgery, with the downtime that comes with it. |
Hyaluronic acid is where I start because it’s the conservative option and it can be reversed. If it isn’t right for you, I’ll say so and go through the alternatives with you.
My video on penis enlargement
In episode 3 of my show I go through the enlargement options that exist, what hyaluronic acid filler does, and who it’s for. In the short I explain why most enlargement hacks sold online are scams.
What penile filler costs in Los Angeles
Most published Los Angeles quotes land between $3,500 and $6,000 for one treatment. I’d put the full range at about $2,000 to $10,000 or more, and it’s set mostly by how much filler you want.
The price moves with how much filler you need, how many sessions it takes, whether a touch-up is included, and who does the injecting. Cheap filler in inexperienced hands is rarely a bargain.
My own price is set at launch. I’m not going to charge anybody for an expensive procedure until we’ve at least talked about it, so the consult is free. Filler is cosmetic, so it’s cash pay and insurance doesn’t cover it. Financing is available through CareCredit and Cherry.

What is the average cost of penile fillers?
Most published Los Angeles quotes land between $3,500 and $6,000 for one treatment. I’d put the full range at about $2,000 to $10,000 or more, mostly depending on how much filler you want. My own price is set at launch. The first consult is free, and financing is available through CareCredit and Cherry. Read the penile filler cost guide.
Do penile fillers actually work?
Yes. In randomized trials and larger studies, men gained on average 2 to 2.5 cm of girth, and 89% of the 324 men in one audit were satisfied. I also see added flaccid length, and men tell me sensation and erections get better.
How much girth does filler add?
On average about 2 to 2.5 cm (roughly 3/4 to 1 inch) of flaccid girth in published studies, with a range of about 1.5 to 4 cm. The erect gain is smaller, about 0.8 cm at one year in one study. How much you can add depends on your anatomy and the volume we use.
Does penile filler add length?
Yes, when you’re soft. The added volume gives a fuller, longer hang, and I tell men it makes growers become showers. For men who have lost some length over the years it’s a great option. Erect length is set by the erectile tissue, so if that’s your main goal I’ll walk you through every option, implants included.
How long does penile filler last?
Roughly 12 to 18 months in published studies. Your body breaks hyaluronic acid down slowly, so the result fades gradually. A top-up at the 12 to 18 month mark keeps it.
Is penile filler permanent?
No. Hyaluronic acid is temporary, and an enzyme called hyaluronidase dissolves it if needed. Permanent fillers such as silicone or PMMA can’t be dissolved and often need surgery to remove when something goes wrong.
Is penile filler safe?
In published studies hyaluronic acid filler has a low rate of complications. Most are minor, like lumps, unevenness, gel moving into the foreskin, or infection, and most are fixable. Serious problems are rare and mostly involve self-injection, unknown products or gel placed in the wrong tissue, so choose who injects you carefully.
Is the procedure painful?
Most men find it almost painless. I use a local anesthetic (lidocaine), so you feel the numbing injection and some pressure. Expect some soreness, swelling and maybe bruising for a few days while things settle.
How soon can I have sex after penile filler?
Published protocols ask men to wait about three weeks so the gel settles evenly, and other clinics use two to four weeks. I’ll give you my exact timeline and aftercare instructions at your visit.
How many sessions are needed?
Most men need one treatment session. A short touch-up visit can even out the result, and a top-up at the 12 to 18 month mark keeps it. Filler has no fixed ceiling, so you can build in stages. The limits are what still looks natural and how much you want.
Can the girth enhancement be removed or reduced?
Yes. An enzyme called hyaluronidase dissolves hyaluronic acid. I can use it to smooth a lump, fix unevenness, or remove the filler entirely if you ever want it gone.
Can you show me before and after photos of penile filler?
Not on a public website. To protect my patients’ privacy, I show before and after photos privately during your consult. Online, you can see how results are measured and how they change over time on the penile filler before and after page.
Why should I choose a urologist for penile girth enhancement?
A urologist works every day with the anatomy involved, meaning the nerves, the blood vessels, the urethra and the tissue layers the filler sits between. I also check erections, hormones and conditions like Peyronie’s disease first, and I can manage complications. Of the roughly seven providers doing penile filler in greater Los Angeles, only two are urologists.
Can I get penile filler if I’m on testosterone?
Yes. Being on testosterone doesn’t rule you out, and many of the men I see are on it or thinking about it. I go over your hormones, labs and erections in the same consult, so your filler plan and your testosterone plan fit together. Read about testosterone therapy from a urologist.
Is the first consultation free?
Yes. The penile filler consult is free, by phone or video, and it’s with me. I’m not going to charge anybody for an expensive procedure until we’ve at least talked about it.
Where do you see patients, and do you see men from outside Los Angeles?
Telehealth consults are open to men across California and Georgia from late November 2026. In-person visits in Los Angeles start in December 2026, for men in Beverly Hills, Century City, West Hollywood, Brentwood, Santa Monica and Westwood. The clinic address is confirmed at launch.
