Dr. Jeremy GoodmanBoard-certified urologist
Men’s longevity care in Los Angeles starts with sexual health
I’m a urologist, and with men I start with sexual health, because if your sexual health isn’t good, the rest doesn’t matter. Problems with erections, hormones or fertility often show up years before anything else, and they point to your heart and your metabolism.
The longevity consult is a paid telehealth visit and runs a full hour. Telehealth in California and Georgia from late November. In person in Los Angeles from December.
Written and medically reviewed by Jeremy Goodman, MD, board-certified urologist. Last reviewed .

What a men’s longevity clinic does
A men’s longevity clinic looks for the problems that cut a man’s healthy years short while they’re still early and fixable. For most men that means heart and blood vessel disease, diabetes, low testosterone and prostate disease. At Goodman Longevity I do that workup myself, for men in Los Angeles and Beverly Hills, and I start with sexual health.
Urology is the specialty that covers men’s health, from the prostate and kidneys to erections, fertility and the hormones behind them. I went to Stanford and then to medical school at UCSF, finished my urology residency at the University of Wisconsin, and spent two years in South Georgia as a urologic surgeon doing kidney stone surgery, cancer surgery and men’s health before opening my own practice in Los Angeles.
Before medicine I competed as a bodybuilder and worked as a personal trainer, and I’ve studied nutrition for years. So your plan covers your training and your diet along with the labs and any medicine. Those change your numbers on their own.

Erection problems often start in a man’s 30s, years before anything shows up on a heart scan. Expert consensus says a man with ED and no known heart disease should have his cardiovascular risk checked.5
Sexual health is a prime marker of longevity for men
Erections depend on healthy blood vessels, and hormones and sperm reflect your metabolism and your general health. A lot of clinics write the prescription for the erections and leave it there. I check your heart and your metabolism as well.
Erections and the heart
The blood vessels in the penis are small, so when blood vessels start to fail, erections usually show it first. In the COBRA study of men with chronic coronary disease and ED, the ED came first in 93% of them, on average two to three years before the heart symptoms.1
Age makes a difference too. In a population study of 1,402 men, men in their 40s with ED had a much higher rate of coronary disease later on than men the same age without it.2 In a large prevention trial, men with ED had about 1.45 times the risk of a later heart event, close to the added risk from smoking.3 About 52% of men aged 40 to 70 report some degree of ED.4 So when a man tells me about erection problems, I check his heart as well.
Hormones and metabolism
Low testosterone, low energy, poor sleep, belly fat and ED tend to show up together, and they’re tied to metabolic disease like diabetes. If one of them is off, I look at all of them.
One low testosterone number isn’t a diagnosis. The American Urological Association calls for symptoms plus two early-morning testosterone tests on separate days, and then a look at why it’s low.6
Sperm and fertility
If you want children, now or later, that changes the plan. Testosterone therapy shuts down sperm production, and the AUA says it shouldn’t be prescribed to men who are trying to conceive.6
There are options that protect fertility, and a semen analysis belongs in the workup before you start anything. I ask about kids at the first visit.
How a longevity assessment works
The longevity consult is a paid telehealth visit with me. I keep the number of patients low on purpose, so every visit runs a full hour and nobody gets rushed.
The conversation
We spend a full hour by video. I go through your history and family history, your medications and supplements, your sleep and stress, and how you train and eat. I also ask the questions you may not have asked anyone, about erections, libido, fertility and size. Nothing is off the table.
The labs
I pick the bloodwork for your age and goals, so you don’t get a one-size panel. Testosterone is drawn early in the morning and repeated if it’s low, the way the AUA guideline asks.
The plan
I go through every number with you in plain English and put the plan in order. Sleep, training and nutrition come first, then treatment where it’s warranted. If something needs another specialist, I’ll tell you.
The long run
We recheck the labs that count on a schedule and adjust as we go, and you keep the same doctor. Members can text me and my team between visits.
What the labs usually cover
Every test is there for a reason you can name. I order what fits your age, symptoms and goals, and I skip the expensive extras that don’t change a decision.
If you only want labs, you can start there and have me read them. The price is set at launch.
| Test | Why it’s there |
|---|---|
| Total testosterone (early morning, twice if low), free testosterone and SHBG as needed, LH, prolactin if LH is low | Confirms low testosterone and finds the cause6 |
| Complete blood count | A baseline, and the main safety check once you’re on testosterone |
| PSA | Before testosterone in men over 40. Otherwise a shared decision, usually from 55 to 696,8 |
| Lipid panel, Lp(a) once in adulthood, apoB when it helps | Your inherited and current cardiovascular risk7 |
| HbA1c or fasting glucose | Diabetes screening, since ED and low testosterone are linked to metabolic disease |
| Kidney and liver function | A baseline before any medication |
| Estradiol | Only with breast symptoms or once you’re on therapy |
| Semen analysis | For men who want children, before or while considering testosterone |
Adding a physical exam
The in-person evaluation in Los Angeles adds a full physical exam, and we can cover hormones, filler or anything else in the same visit. In-person visits start in December. The fee is set at launch. Join the in-person list

Training, sleep and stress
Whatever you come in with, I’ve been there. I competed as a bodybuilder, and I know what hard training and strict diets do to the body, along with the shortcuts men take to get there.
I can talk protein, recovery and sleep with you as someone who has done it, and then read your labs as a urologist. If a supplement doesn’t have the evidence behind it, I’ll tell you.
Videos on sleep, stress and hormones
I post straight answers on YouTube. A weekly podcast about men’s sexual health starts soon on the same channel.
How this differs from a TRT clinic
Most men’s clinics in Los Angeles are TRT mills run by nurse practitioners. They start hormones and can’t handle much beyond that. As a urologist I treat the whole system those hormones act on. If your erections or your prostate need attention, or you want to protect your fertility, you’re already in the right office.

| Topic | Typical TRT clinic | Goodman Longevity |
|---|---|---|
| Who you see | Usually a nurse practitioner | Me, a board-certified urologist, at every visit |
| Where it starts | A testosterone level and a prescription | Your symptoms, two early-morning tests, and the reason behind a low number |
| Fertility | Often an afterthought | Asked at the first visit, with options that protect fertility if you want kids |
| What’s covered | Hormones, and not much beyond them | Hormones, erections, prostate, fertility, heart and metabolic risk, and penile filler |
| Visit length | Short visits and a lot of patients | A full hour, and nobody gets rushed |

Questions men ask me about longevity

What does a men’s longevity clinic do?
It looks for the problems that cut a man’s healthy years short while they’re still early and fixable. That usually means heart and blood vessel disease, diabetes, low testosterone and prostate disease. At Goodman Longevity I do that workup myself as a board-certified urologist, and I start with sexual health.
Why is sexual health a marker of longevity?
Erections depend on healthy blood vessels, and hormones and sperm reflect your metabolism and your general health. In a study of men who had both heart disease and ED, the erection problems came first in 93% of them, by about two to three years. Sexual health is often the earliest warning a man gets.
Is erectile dysfunction in my 30s or 40s a warning sign?
It can be. Erection problems often start in a man’s 30s, and in younger men they’re linked to a much higher risk of coronary disease later. Expert consensus says a man with ED and no known heart disease should have his cardiovascular risk checked, so I treat the erections and I check your heart.
What labs are included in a longevity assessment?
I pick the panel for your age and goals. It usually covers early-morning testosterone and the hormones that explain it, a blood count, PSA when it’s appropriate, a lipid panel with Lp(a) and sometimes apoB, HbA1c or fasting glucose, kidney and liver function, and a semen analysis if you want children.
How is this different from a TRT clinic?
Most TRT clinics are run by nurse practitioners who start testosterone and can’t handle much beyond that. I’m a board-certified urologist. I confirm low testosterone properly and look for the cause, I protect your fertility if you want children, and I also treat erections, the prostate and the rest of men’s health. Read about testosterone therapy from a urologist.
Will I end up on testosterone?
Only if you need it. We start with sleep, training, nutrition and stress, which move the numbers on their own. Testosterone is an option when your symptoms and two early-morning tests both point to it. If you’re already on it, I’ll go through your labs and your protocol with you.
Can I do the longevity consult by telehealth?
Yes. The longevity consult is a telehealth visit for patients in California and Georgia from late November 2026. In-person visits in Los Angeles, with a full physical exam, start in December 2026.
Is longevity care covered by insurance?
No. Goodman Longevity is cash pay, which is what lets every visit run a full hour. Financing is available through CareCredit and Cherry. If you have questions before you book, text my team.
